New Patient Registration

Meddygfa Sarn welcomes new patients from within our boundary area. If you would like to register with the Practice, please either attend the surgery to collect the registration form together with the new patient health questionnaire or download the forms using the links on this page to print and complete at home.  It is very important that you complete these forms fully.
Please make sure you have your NHS number when completing the new registrations forms.  Your NHS Number is a 10-digit number and it is unique to you.  It helps healthcare staff and service providers identify you correctly and match your details to your health records.

You may be required to provide proof of address.

Before registering and completing forms, please ring the Receptionist at Meddygfa Sarn on 01269 860348 to check if you are living within our practice area. 

We offer all new patients a simple health check – height, weight, BP, urine test etc.  By attending this appointment it allows us to record certain data including your medical history, allergies and family history.  Having this information will make medical staff better aware of your needs.

When returning your completed form to the surgery you will be advised to make an appointment with the Practice Nurse or Health Care Assistant for a new patient health check.  The Practice does not discriminate on the grounds of age, gender, disability, race, religion, sexual orientation, social class, appearance or medical condition.

Please note: patients are registered with the practice, not an individual GP.  However, you can at any time express a preference for a particular doctor and we will do our best to respect your choice.  Please be aware, however, that not all doctors in the practice provide all services and specific doctors may not be immediately available.

Inform the receptionist if you have ever been a member of the Armed Forces when registering.  

If you would also like to give your consent for appointment reminders or health promotions, please download the text messaging consent form and return it to the surgery.

Please make every effort to ensure that your name, address and telephone/mobile numbers are kept correct.

As a Practice we are committed to providing a safe, comfortable environment for our patients.  If you would like a chaperone during your consultation, please ask the doctor.  A Practice Nurse or Health Care Assistant will usually act as your chaperone, but on rare occasions a Receptionist may be asked to chaperone

Disabled Access.

The surgery has access facilities for physically and sensory impaired patients. A hearing loop is available and a wheelchair is also available on-site.

Please let us know in advance, if you will need additional assistance to access our building. We regret that we do not yet have automatic doors, though we will be happy to help you.


Should any patient with hearing, visual or any other disability require assistance, please contact our reception team to discuss.

Medication List.

Upon registration, please provide us with a copy of your last prescribed medicine list (or empty boxes of medication) so we know what should be on your repeat prescription list.  

Temporary Residents.

You are able to see a doctor anywhere in the UK, if you are away from home and in need of medical help.  You can do this by simply asking to see the nearest doctor, as a temporary resident.  We will be happy to see any relatives or friends staying with you, if they need a doctor.  

Change of Details.

If you change your name, address or telephone number please come into the surgery and fill in a ‘Change of Details’ form. If you move without telling us we might write to, visit or telephone the wrong house. It is particularly important that we have your correct telephone number in case we need to contact you urgently. 

If this change involves moving outside the practice area (as shown on the map) we would be unable to continue offering our medical services.  Therefore you would be required to register with another Practice.

Remember to tell other agencies about your change of details too – dentist, hospital clinic (contact the hospital Medical Records Dept. by telephone).
We will be happy to see any relatives or friends staying with you, if they need a doctor.
